    Firefighter Haudenschild died from injuries sustained when the fire apparatus he was operating crashed as he was responding to a reported grass fire. Haudenschild was reportedly not wearing seat restraints and passed away at the scene of blunt force trauma due to the motor vehicle crash.

    Upon returning to quarters after helping to put out a small kitchen fire in the South Side Beverly neighborhood of Chicago, Firefighter Patmon fell ill while cleaning equipment. Patmon was treated by fellow first responders and transported to the hospital where he passed away from an apparent heart attack.     David Mitchell Tatum of the White Oak Fire Department in White Oak, NC has died as the result of an on-duty incident that occurred on 2012-11-04

    Initial Summary:  
    Lieutenant Tatum became sick at the scene of an emergency response but did not request treatment at that time. Later in the day, he began having trouble breathing and became unconscious. Tatum was treated and transported to Bladen County Hospital and then transferred to Cape Fear Valley Hospital in Fayetteville where he passed away from a cause still to be determined.

        OK another weird coroner case–and call for ventilation

        DENVER - Officers responding to complaints of a foul smell in a south Denver apartment building made a gruesome yet bizarre discovery earlier this month.
        Inside the apartment they found a woman's decomposing body slumped on a chair. Next to the body was a Great Dane, which appeared to be guarding the body, according to a search warrant affidavit obtained by 7NEWS.
        The woman's husband was also inside, sitting on a chair in the same room, the affidavit said.
        When they asked the man how long the woman had been dead, he replied, "She's not dead. She's fine," according to the affidavit. The man appeared to be either intoxicated or "out of sorts," the affidavit said.
        The deceased woman has been identified as 32-year-old Michaela Beth Ford. The man was identified as her husband Steven Ford.
        Neighbors said they wondered what was up when they noticed a bad smell coming from Apt. 308.
        "The hallway kind of started to smell weird, but it's like, you live in an apartment building. A million different people live around here," neighbor Andrew Cosgrove said.
        Police said the woman appeared to have been dead more than 10 days and her body was in an advanced state of decomposition.
        A maintenance manager said he saw the husband coming and going during the previous two weeks, going to the liquor store, but had not seen the wife.
        Ford has not been charged and police are apparently continuing their investigation, although they had no comment on the case on Sunday.

        US Fire Administration Releases Report on Intentionally Set Fires in Residential Buildings (2008-2010)

        The U.S. Fire Administration issued a special report today examining the characteristics of intentionally set fires in residential buildings. An estimated 16,800 intentionally set fires in residential buildings occur annually in the U.S. These fires result in an estimated average of 280 deaths, 775 injuries and $593 million in property loss each year.

        Do you have a code of Ethics, is it followed and supported by you city , fire district or special district? It’s not something that should be taken lightly but a code to follow as your guideline to follow. It’s a mind set that should be in every firefighter from day one. Followed from the top down and the bottom up.

        U.S. Fire Administration Acknowledges the New Fire Service Ethics Statement

        EMMITSBURG, MD – Today’s fire service leaders face a variety of challenges managing budgets, personnel, and programs. Occasionally, ethical issues emerge for which there are no easy answers. To provide guidance to address these complex questions, the National Society of Executive Fire Officers (NSEFO) and Congressional Fire Services Institute (CFSI) jointly have released a firefighter code of ethics that can be adopted by local fire and emergency medical service organizations. The ethics statement is the result of almost two years of effort by the Board of Directors of NSEFO.

        In acknowledgement of the importance of this first of its kind fire service ethics statement, the U.S. Fire Administration (USFA) has posted the statement throughout the National Fire Academy’s (NFA) facilities and will now include the statement in all course materials distributed to NFA students.

        USFA report along with Consumer product Safety Commission

        Kitchen Fires No. 1 in New Report

        In recognition of Fire Prevention Week, the U.S. Consumer Product Safety Commission and the U.S. Fire Administration are providing new statistics on fires in American homes and urging consumers to install smoke alarms in their homes and check to make sure all smoke alarms are working properly. It is also vitally important to develop and practice a family fire escape plan. The top cause of fires in the home is cooking equipment, accounting for an estimated 147,400 or 40 percent of residential fires each year between 2008 and 2010. Cooking was also associated with the largest percentage of fire-related injuries, an estimated average 27.4 percent or 3,450, in the home.

        Major wildfire in Kittson and Marshall Counties could have been arson

        Arson likely cause of County 27 Fire (Karlstad), reward offered

        Fire investigators have determined that the probable cause of a major wildfire in Kittson and Marshall Counties in northwestern Minnesota was arson.  An investigation is ongoing by the Kittson County Sheriff, the Marshall County Sheriff and the Minnesota Department of Natural Resources.They are offering a reward of up to $6,000 for information leading to identification of the arsonist.

        Anyone with information on the County 27 fire is encouraged to contact either the Kittson County Sheriff at 218-843-3535 or the Marshall County Sheriff at 218-745-5411.  For those who wish to remain anonymous, call the Arson Tip Line at 1-800-723-2020.

        The wildfire started Sept. 30, at about 2 p.m. in Marshall County north of the intersection of County Road 27 and 250th Avenue NW.  By the next day, it had grown to 500 acres.  On Oct. 2, there were Red Flag Fire Warnings for western Minnesota and states and provinces to the west and north.  Temperatures were in the 70s, relative humidity exceptionally low – in the teens and sustained winds of 35 mph with gusts to 45 mph.

        The Red Flag fire danger conditions led to fast moving fires that burned with extreme intensity.  The County 27 Fire spread from Marshall County to Kittson County, burned 4,440 acres, led to the evacuation of part of the city of Karlstad, damaged dozens of structures and destroyed
        11 homes.

        The coordinated efforts of local fire departments, wildland firefighters, sheriff’s offices, emergency managers, and the National Guard were instrumental in saving most of the homes in Karlstad.

        Fires started by arsonists cost Minnesota taxpayers millions of dollars each year.  The cost to those who lost homes is immeasurable.  Please help stop arson.

        OLYMPIA, Wash. (AP) — It's a complicated story, but a Thurston County, Wash., man says he managed to shoot himself in the head with a .22-caliber rifle while fishing for salmon.A sheriff's report says the man told deputies he fired a shot into the Deschutes River on Sunday afternoon but the bullet ricocheted off a rock and hit him in the temple. When he scratched the spot with his finger, he says the bullet fell out of his head and into the river.
        The tale came to light although the man went home and refused to tell his girlfriend how he had been injured. After he left again, she called 911. When deputies pulled over the man's pickup truck, he told them what happened.
        The Olympian reports that the man was treated at an area hospital and released. The newspaper says the man was not cited for any violations.

        Couple of LODD notices from USFA

        Notice of Firefighter Fatality: Riverview, MI

        John Grabowski of the Riverview Fire Department in Riverview, MI has died as the result of an on-duty incident that occurred on 2012-10-01

        Initial Summary: 
        On Sunday, September 30, during his shift, Lieutenant John Grabowski participated in fire department training. His shift ended that day at 1800hrs. The following morning, Lieutenant Grabowski traveled to his second job. At approximately 0803hrs, Lieutenant Grabowski was found collapsed in the parking lot of his second employer. The second employer?s emergency response team tended to Lieutenant Grabowski and performed CPR. Advanced life support also responded to the scene where they treated Lieutenant Grabowski and transported him to Henry Ford Wyandotte Michigan hospital. Efforts to revive Lieutenant Grabowski at the hospital were not successful and he passed away within approximately one hour of being found in his employer?s parking lot. The official cause of death has not yet been determined.

          Justin Townsend of the Dagsboro Volunteer Fire Department in Dagsboro, DE has died as the result of an on-duty incident that occurred on 2012-09-27

          Initial Summary: 
          Dagsboro Volunteer Fire Department was dispatched for an outside fire in a wooded area on East Clogg Drive just outside town limits. While en route to Station 73 in a personal vehicle, Junior Firefighter Justin Townsend was fatally injured in a motor vehicle crash on Pepper Road, in the area of Jay Patch Road. No details of the accident are available at this time. The Delaware State Police Crash Reconstruction Unit is currently conducting an investigation into the fatal incident.           Fall begins this Saturday, and with it comes cooler temperatures and the resulting seasonal increase in the number of home heating fires. To help Americans understand the nature of the heating fire problem, the U.S. Fire Administration (USFA) today issued a special report: Heating Fires in Residential Buildings (2008-2010). Heating is the second leading cause of all residential building fires. Home heating fires are preventable! USFA reminds everyone to maintain heating equipment and chimneys by having them cleaned and inspected annually by a qualified professional.

            Neal W. Smith of the Atascocita Volunteer Fire Department in Atascocita, TX has died as the result of an on-duty incident that occurred on 2012-09-15 

             

            Initial Summary:
            On Saturday, September 15, during a smoke diver class designed to train firefighters advanced survival skills in full gear, Captain Neal Smith passed out from a heat-related illness. He was transported to a hospital during which CPR was administered. Captain Smith remained in critical condition until Monday, September 17, when he succumbed to his injuries at approximately 6:30 p.m. Doctors reportedly determined that Captain Smith's body temperature was more than 106 degrees.

            Palmer Township, Pa-- Palmer Townships Fire Commissioner pulled out of the station and confirmed the worker at 3029 Old Nazareth Pike this afternoon. First arriving ladder 2721 grabbed the plug at Swanson Street and made a short lay in, crews stretched a line and made entry to the fire which was blowing out both gable end vents in the attic. Palmers engine arrived next and took roof operations while crews from Nancy Run set up the FAST assignment, Hecktown's crews stood by for manpower. All in all first arriving responders rescued 2 pets and had the fire knocked with in 5 minutes of arrival. All companies were held on the scene to assist with some minor overhaul, negative injuries were reported.

            Allentown, Pa-- Companies working on the 4th alarm in the 800 block of N. 8th Street. Nine homes were reported effected by the fire and five firefighters were transported to the hospital.
            Firefighters arrived to find heavy fire on the first floor of a 3 story M/O/R, a 2nd alarm was transmitted. 4A requested a supply line a 2nd line and make entry to the fire floor. Truck 2 set up on the alpha side and went to work with roof operations. Companies had issues with the alley behind the row limiting access to engine and ladder companies.
            Crews went to work on division 1 with heavy fire on the 1st floor dining room and extension rolling to the 2nd floor. Updates from the outside were heavy fire rolling out the rear, with this Chief 40 requested the 3rd alarm.
            The fire eventually took the 2nd and 3rd floors blowing out the roof with extension to multiple exposures, cmd requested the 4th alarm evacuated all companies and went to ladder pipe operations and multiple lines from the exterior.

            Fire Tornado Caught On Tape In Australia Fire Twister

            THERE'S something mean and magical about Australia's Outback. An Alice Springs filmmaker captured both when a whirlwind of fire erupted before his eyes.
            Chris Tangey of Alice Springs Film and Television was scouting locations near Curtin Springs station, about 80km from Ularu, last week when confronted by a fiery phenomenon.
            He had just finished his tour of the station when workers encountered difficulties with a grader. So he went to help them.
            A small fire was burning in nearby bushland, so Mr Tangey decided to start filming.
            He caught the sight of his life.
            A twister touched down on the spot fire, fanning it into a furious tower of flame.
            "It sounded like a jet fighter going by, yet there wasn't a breath of wind where we were," he told the Northern Territory News.
            "You would have paid $1000 a head if you knew it was about to happen."
            The column of fire danced about the landscape for about 40 minutes, he said, as he and the station workers stood transfixed.
            There was talk of making a quick getaway, Mr Tangey said. But everyone was too hypnotised to feel scared - and he continued furiously filming.
            "The bizarre thing was that it rarely moved," he said.
            "These things just stood there because there was no wind to move them ... but it was flickering incredibly fast."
            Darwin weather forecaster David Matthews said small twisters were common in isolated areas. But the fiery vortex was highly unusual.
            "The flames would have assisted by trying to suck in air and that could have helped generate those circular winds," Mr Matthews said.

            U.S. Fire Administration Acknowledges the New Fire Service Ethics Statement

            To provide guidance to address complex ethical questions, the National Society of Executive Fire Officers and Congressional Fire Services Institute jointly have released a firefighter code of ethics that can be adopted by local fire and emergency medical service organizations. In acknowledgement of the importance of this first of its kind fire service ethics statement, the U.S. Fire Administration has posted the statement throughout the National Fire Academy's (NFA) facilities and will now include the statement in all course materials distributed to NFA students.

            New Report From USFA On Clothes Dryers

            U.S. Fire Administration Releases New Report on Clothes Dryer Fires in Residential Buildings

            The U.S. Fire Administration issued a special report today examining the characteristics of clothes dryer fires in residential buildings. The report, Clothes Dryer Fires in Residential Buildings (2008-2010), shows that failure to clean is the leading factor contributing to the ignition of these fires.

            According to the report:

            • An estimated 2,900 clothes dryer fires in residential buildings are reported to U.S. fire departments each year and cause an estimated 5 deaths, 100 injuries, and $35 million in property loss.
            • Clothes dryer fire incidence in residential buildings was higher in the fall and winter months, peaking in January at 11 percent.
            • Failure to clean (34 percent) was the leading factor contributing to the ignition of clothes dryer fires in residential buildings.
            • Dust, fiber, and lint (28 percent) and clothing not on a person (27 percent) were, by far, the leading items first ignited in clothes dryer fires in residential buildings.
            • Fifty-four percent of clothes dryer fires in residential buildings were confined to the object of origin.

                    Sunday, June 10, 2012, at approximately 1100hrs, Assistant Chief Suggs had responded to an emergency call. After returning home, he collapsed at about 2100hrs that evening. He was taken to the hospital where he passed away on Monday, June 11, 2012, at approximately 0100hrs from an apparent heart attack.                     This video proves that even a small fire can cause major headaches for firefighters, thankfully Lake Station as usual had an awesome response time. If they were another five minutes out the building would have surely been involved other than some melted siding. The call came in just after 11am for some type of fire next to the food pantry at Columbus Park. On arrival a lawnmower was for engulfed in flames. Crews quickly stretched a line and knocked the fire. Nobody was injured at the fire scene. Once again, awesome job by LSFD

                    At a McDonald's in Coachella (California), a truck with propane tanks in the bed catch on fire and end with a BOOM. Luckily, everyone made it out of the truck in time. They are, however, experiencing second degree burns.

                    A video on YouTube shows a massive pickup truck explosion at a McDonald's drive-thru in California.

                    The blast cause the truck’s roof to buckle and the tailgate to fly off and hit another vehicle. The fire engulfed the truck, scorched part of the drive-thru and damaged the restaurant’s roof.

                    Recently, when I went to McDonald's I saw on the menu that you could have an order of 6, 9 or 12 Chicken McNuggets .

                    I asked for a half dozen nuggets.

                    'We don't have half dozen nuggets,' said the teenager at the counter.

                    'You don't?' I replied.

                    'We only have six, nine, or twelve,' was the reply.

                    'So I can't order a half dozen nuggets, but I can order six?'

                    'That's right.'

                    So I shook my head and ordered six McNuggets

                    (Unbelievable but sadly true...)

                    TWO

                    I was checking out at the local Walmart with just a few items and the lady behind me put her things on the belt close to mine. I picked up one of those 'dividers' that they keep by the cash register and placed it between our things so they wouldn't get mixed.

                    After the girl had scanned all of my items, she picked up the 'divider', looking it all over for the bar code so she could scan it.

                    Not finding the bar code, she said to me, 'Do you know how much this is?'

                    I said to her 'I've changed my mind; I don't think I'll buy that today.'

                    She said 'OK,' and I paid her for the things and left.

                    She had no clue to what had just happened.

                    A woman at work was seen putting a credit card into her floppy drive and pulling it out very quickly.

                    When I inquired as to what she was doing, she said she was shopping on the Internet and they kept asking for a credit card number, so she was using the ATM 'thingy.'

                    FOUR

                    I recently saw a distraught young lady weeping beside her car. 'Do you need some help?' I asked.

                    She replied, 'I knew I should have replaced the battery to this remote door unlocker. Now I can't get into my car. Do you think they (pointing to a distant convenience store) would have a battery to fit this?'

                    'Hmmm, I don't know. Do you have an alarm, too?' I asked.

                    'No, just this remote thingy,' she answered, handing it and the car keys to me. As I took the key and manually unlocked the door, I replied, 'Why don't you drive over there and check about the batteries. It's a long walk....'

                    PLEASE just lay down before you hurt yourself !!!

                    Several years ago, we had an Intern who was none too swift. One day she was typing and turned to a secretary and said, 'I'm almost out of typing paper. What do I do?'

                    'Just use paper from the photocopier', the secretary told her. With that, the intern took her last remaining blank piece of paper, put it on the photocopier and proceeded to make five blank copies.

                    A mother calls 911 very worried asking the dispatcher if she needs to take her kid to the emergency room, the kid had eaten ants. The dispatcher tells her to give the kid some Benadryl and he should be fine. Then the mother says, 'I just gave him some ant killer......'

                    Dispatcher : 'Rush him in to emergency!'
